KVKK Request Form

Under Law No. 6698 on the Protection of Personal Data (the “Law”), data subjects (the “Applicant”) — defined as natural persons whose personal data is processed — are granted the right under Article 11 to submit certain requests concerning the processing of their personal data to data controllers. Pursuant to the first paragraph of Article 13 of the Law, applications regarding these rights must be submitted in writing to the data controller KRY Loyaltech Teknoloji Proje Geliştirme A.Ş.

Application methods:

In writing (wet-signed): print out this form and submit it wet-signed to “Kavacık Mah. Muhtar Sk. No: 5 İç Kapı No: 4, Beykoz / İstanbul” via notary public, by registered mail with return receipt, or in person.
By e-mail: send it to info@loyexo.com from the e-mail address you have notified to and registered with the Company.

Your application will be concluded “as soon as possible and within thirty (30) days at the latest” from the date it reaches the Company, depending on the nature of the request. Where your application additionally entails a cost, the fee set out in the Communiqué on the Procedures and Principles of Application to the Data Controller will be charged (first 10 pages free; TRY 1 for each subsequent page; where a recording medium is provided, the cost of the medium).

The Company reserves the right to request additional documents/information for identity and authorisation verification (e.g. a copy of your ID card, passport or driving licence). The Company accepts no liability where the information provided in the form is inaccurate or not up to date, or in the event of unauthorised application.

You may complete the form on screen and print it, or print it blank and fill it in by hand.

The Company reserves the right to make changes in accordance with any decision of the Board or future legal regulations regarding application procedures.
